To participate in the Halftime Performance:

• Submit the online registration by Friday, September 21, 2018

• You will receive receive an email with your completed registration. Please print 2 copies, one to keep and one to mail in with your payment.

• Print and complete the three (3) forms available in the information packet on the website:

• 1- Release and Consent Form
• 2- Rules and Regulations Form
• 3- Release of Liability/Medical Consent Form

• Mail your registration page, three forms, and the registration fee WITHIN 7 DAYS of registration.


• Registration Fees must be paid in full NO LATER THAN September 21, 2018.


• Make check or money order payable to: American All-Star, LLC. Mail forms and fees to:

Your space in the Halftime Show is not reserved until the forms and payment are received.
Once your registration papers and non-refundable fees are received, you will receive an email confirmation with additional performance details.
